Care Manager is a professional who works in various healthcare or social service settings to coordinate and manage care services for individuals in need. Their primary responsibility is to ensure that clients receive appropriate and timely care to improve their overall well-being.
• Conduct thorough assessments of patient’s physical, emotional, and social needs to determine the level and type of care required.
• Provide education and support to patients and their families, empowering them to make informed decisions about their care and promoting self-management whenever possible.
• Regularly speak with patients and develop comprehensive care plans based on patient’s needs and preferences, incorporating input from healthcare professionals, family members, and other relevant parties.
• Arrange various care services, such as medical treatments, therapy sessions, home healthcare visits, and social support programs, to ensure they are delivered effectively and efficiently.
• Respond accurately and professionally to medical information enquiries received via phone, email or chat regarding pharmaceutical or device products.
• Ensure the medical accuracy of advertising, scientific and promotional materials of the company.
• Regularly assess clients' progress and make necessary adjustments to care plans, ensuring that services are meeting their evolving needs and goals.
• Maintain accurate and up-to-date records of clients' assessments, care plans, progress notes, and any relevant information, adhering to confidentiality and privacy guidelines
RequirementsAny Doctor, Nurse, physician can apply
1+ years of experience working in providing remote patient care, US Healthcare IT industry preferable
Proficient with Healthcare Management Systems and Microsoft Office Word and Excel
Excellent written and verbal communication
Command in English, Hindi and Gujarati
Strong understanding of healthcare systems, medical terminology, and community resources is essential.
Demonstrate an understanding of standard clinical procedures, laws and regulations.
Proficiency in using electronic health records (EHR) systems, care management software, and other relevant technology tools is increasingly important in modern care management practice.
